On-Demand Video Bridge Service Request Form

On-Demand video bridge conference profiles and changes to existing profiles can be requested by contacting the Information Technology Services Call Center Services at 814-863-8906 or by completing the following form. Requests will be processed within two business days via a confirmation email sent to the designated conference chairperson.

Please note: This form is ONLY for requesting new or changing On-Demand Video Bridge profiles. For ITS scheduled video bridge reservation requests please use the Video Bridge Reservation Form .

* Name of Conference:		This name will be programmed as a voice prompt on the Video Bridge to identify the conference being entered.
* Maximum number of sites:	Limited to a maximum of 10 sites.	This is the maximum number of sites that will join the conference.
* Maximum duration of the conference:	Hrs: Mins: Limited to a maximum of 2 hrs and 30 minutes	The maximum duration of the conference in hours and minutes.
* Conference Mode Layout:		The conference mode layout default is Auto Layout (adapts to # of connected sites). See The Penn State Video Bridge for conference mode layout information. If a fixed conference mode layout is required, please specify.
* Conference chairperson:		The conference chairperson is the main point of contact for use of the specific conference.
